To enter Project Uplift presented by the
Skin & Vein Center, please complete the
following information.
Submissions can also be sent

mail at Project Uplift, 305 N Leroy, Fenton,
MI 48430.
Only one entry per person. Submissions not
chosen during a particular month will
continue to be considered for up to six
months. After which, contestants who have
not been selected may re-submit their entry
for consideration.
Entrants selected must be medically fit to
undergo this procedure and able to have the
procedure performed at one of the Skin &
Vein Center locations in Fenton, Garden City
or Sterling Heights, Michigan. Recipients
also must be able to provide their own
transportation to and from the procedure.
Entrants must provide their first name, last
name, date of birth, email address, phone
number, a recent photo and an explanation of
how either they, or someone they love, would
benefit from a Mini-Facelift.
One recipient will be selected on the first
Monday of each month. Selections will be
chosen by the Skin & Vein Center staff.
Employees and family members of employees of
Skin & Vein Center are not eligible to win.
Entrants must be at least 40 years of age at
time of entry.
